you could have gone to the training department themselves and pointed out your findings
The training dept consisted of one guy, at one stage a direct entry from the military was appointed to the role and returned to the military a matter of months later, reason I presume, from interaction with him, I was under the impression his disgust at standards and inability to affect change may well have been the reason. As said previously, compliance with regs or ops manual was off the table and being staff you were subject to a yearly appraisal which set your salary. As to safety, the operation never had an accident in my 27 years, and management used that as a reason that "we are doing everything right". Normalisation of deviance I'd assume you're familiar with. Fact is major events always occurred in benign circumstances, or local knowledge, experience and skill brought about a safe conclusion.
