Format for General flight test report

Hello friends,

I am about to go for my recency to US.
Can anyone please let me know in what format i should get the general flight test report printed?
Also whether it needs to be printed on the schools letter-head or not.

I appreciate your help..

It needs to be written in Morse Code, as this is the ICAO standard. You should also use Landscape-orientation B4 size paper, and it must be printed on an ink-jet printer (not laser).
Do not fold or crease the paper - mail it to the FAA in a cardboard mailing tube, sealed at both ends, and ensure a packet of silica gel is enclosed to absorb moisture.
