There is a reason that you can only do a first class medical in Kemayoran Baru, Jakarta. Not entirely sure what that reason is, but it is a reason nonetheless.

I did my last class 1 renewal at Surabaya (organized by the company), but would have preferred to have had it done at the aviation medical centre in Kemayoran as you get the results and certificate on the spot there.
