And it seems there is some contradiction as to who is to blame.. Poor guy, judging by that he recovered just before impact.

It's hard to reconcile:

The board president found, by clear and convincing evidence, the cause of the mishap was the [pilot's] failure to recognize and initiate a timely dive recovery due to channelized attention, breakdown of visual scan, and unrecognized spatial disorientation,"
...with the USAF COS statement that the Board did NOT blame the pilot.

If it was hypoxia, which is the only conclusion if he's exonerated, why is there any confusion? It can only be the politics of a multi $$$$ project.

To not at least cite the loss of the bleed air and thus the OBOGS as a major contributory factor in the original investigation really taints it for me. The pilot (wearing a bulky winter suit for the first flight of the season) had very little time to pull the apple. It will be interesting to see if the IG investigation finds the original board was under real or percieved pressure to not find mechanical fault. On such a complex jet it would seem automatic emergency air should come on at any sign of bleed air or OBOGS failure.

So if bleed air to air separators for OBOGS gets shut off by a fire , then presumably on aircraft with OBIGGS (onboard inert gas generation system) for fire prevention would lose their supply too, just when it is most needed. Great!
