F6442 or F6000 appraisal scoring matrix

I apologise for this thread not actually being about military aviation but does anybody have a copy of the old F6442 or F6000 appraisal scoring matrix. The form was blue in colour. I thought I had an old copy but can’t find it anywhere.

If they do, is there any chance they could scan it and email it to me please.
