Transfer of EASA Medcial form one EASA state to another?

Hi,

was wondering if it's possible to transfer an EASA Medical which was issued by EASA sate A (for simplicity) to EASA state B (without undergoing a full exam again)? Any advise appreciated - thanks!

Isn't it possible to just use your old EASA Medical in the new EASA country?
