Medical License Conversion

I have a current EASA Class I medical license with a OML and TML restriction due to a myocardial infarction three years ago.

My question is: Can I apply for a conversion to a medical license outside Europe or do I have to apply for an initial medical license?

The reason for my doubt is that my condition is unfit to apply for a initial medical license but for fit for a renewal.
