Eye Test Form

Hi



Sorry for covering an old topic but some while ago someone posted a link to a document which was from the CAA which was to be filled in by your optician when going for an eye test..... I have had a look around but can’t find it again....can some one please repost me the link so I can download it and take to my eye test next week. It was a document for flight crew detailing what their prescription was and how much astigmatism they had etc.
