Resume - just one question

One of the jobs I want to apply for, ask for a photo to be included.
My resume is without a photo. Should "add" photo to resume, or should I add it separately on the e-mail.
And should it be headshot (like passport photo), or full-height? In uniform or "casual/dress"?

Unless it states otherwise, attach (or incorporate) a Passport style, good quality, colour photograph to the CV. The photograph should be a head and shoulders shot.
