An EASA Form 4 is used for a state regulator (CAA in the UK) to formally determine someone's suitability to hold a nominated post in an organisation - e.g. some airworthiness review staff, accountable managers etc.
Usually a form 4 is required for each job and is valid only for that job, so to ask for form 4 approval up front is a bit of a nonsense unless the company wants to know that you could be approved easily, having held a previous approval.
A form 4 is not required for flight crew under normal circumstances.
