The UK CAA will accept flight tests conducted by a (CURRENT) examiner from other JAA states. This is not, necessarily, true the other way around.

The examiner should sign your licence ratings page (assuming your rating has not expired by more than five years [so six years from last successful test date]) and complete Section 6 of CAA form 1119.
http://www.caa.co.uk/docs/33/FORSRG1119.pdf

YOU should complete Sections 1 and 2 - then send the form to the CAA at the address at Section 6.

Ignore Section 5 - as there is NO FEE payable to the CAA (just to the examiner).
