Apparently the captain of the Air France crash was sleep deprived.

1 hour of sleep prior to a long flight??...who cares about fatigue, it's not important....but should you dare to have a blood alcohol level of .02...

It would be interesting to know if the Commander had a private life insurance policy and if the payout was refused on the basis of sleep deprivation. That would change the face of FTL's.
