MU A320 had push back incident in VHHH

Hi, just heard from another forum that an MU A320 had its noise wheel 90 degrees turned and stuck...think this is similar to the jetBlue one. Does it have anything to do with the design or is it the pilots' fault?

I think this was down to human error and mis communication as i beleive the park brake was Set during the push. Not the first time Park brake been set while moving remember their 340-600.

I doubt it was the parking brake, unless they set it mid-puashback, as they managed to go far enough back that they were almost totally on the taxiway before it happened....if the brakes were on the whole time I think someone would have noticed. The A346 landed with brakes on, so before anyone noticed or could do anything an incident had happened.
