EASA SFE(a5)

A colleague recently revalidated his IAA (Irish) SFE(a5).
Everything was correctly done and the IAA re-issued his SFE Certificate; but WITHOUT the (a5).
Possibly just a clerical error, but proving very difficult to rectify.
Any other Examiners had a similar problem?
