A "Beat-up" Checklist

A while back I saw a printed "Beat-up" checklist that was published by someone like the FAA. I can't find it anymore. Does anyone have it or can point me to it.

It had things on it like, Will current , Family informed, Insurance cancelled............

Any help will be much appreciated.

You forgot:

Undertaker at scene.
Organ donor card in pocket.
Wreckage removal firm on standby.
Runway reconstruction crew available.
Priest to read last rights.
