SITA Graflite flight planning

Hello guys! I was wondering if some of you could help me!
Usually on the last page of SITA navigation flight planning, we have some information about acft fuel tanks. To clarify... the amout of fuel in each tank, so... below this information there are some acronyms such as FOA , CAP , SUP.
Can anyone of you tell me what does these acronyms stands for?

I really appreciate your attention!

These are fields for signatures of FOA (Flight Operations Assistant) CAP (Captain) and SUP (supplier of fuel)
