Cookies just leave information about the last time you visited, as in some preferences or settings you setup on that website, etc.
Clearing your cache would have the effect of slowing down the pages, since your browser will have to go out and get all the graphics for that website again, etc.
There is much more to it than what I just said, but that is a basic overview of what happens.

I clear mine all of the time and reject 99% of those being offered as some are precursors for spyware. It has just about no effect on my browsing as if a cookie is not found it is offered again.
Have a look at cookie wall on www.analogx.com. It give the ability to accept, delete or temporary accept cookies as they arrive.
