Transport Canada Class 1/CAT 1 Medical

Hello Folks!

I am posting this query after scouring Pprune and google for some info but no luck L I will need to do a Canadian class I medical (apparently known as a CAT 1 medical), and I have spoken to the certified medical examiner in my area (certified by Transport Canada) who mentioned that there some forms that I need to bring with me to the exam. I have checked the Transport Canada website and was unable to find these forms. Can anyone here please advise as to where I can obtain these forums online? I would also be grateful if people who have done CAT I medical can share any relevant comments/tips on their experience!

Contact Transport Canada Medical division in Ottawa and request the Civil Aviation Examination Report that is probably the one he means, I take one every time I have my UK medical renewed and my examiner files it out had no problems for near 18yrs that way.
