But to be clear, the ground staff are the last line of defence. The flight crew have several checks that ensure this does not happen, which none appear to have been followed. My reference to Rex was just that I know (at least in the past) that these items were emphasized, so that you'd rarely see an aircraft without the extension attached while on the ground between turnarouns. In this case 6 turbarounds were performed and not one made sure the prop extension strap was secured.

Blaming the ground crew is like blaming GPWS for s CFIT.

What it shows is that training is lacking to make sure crew know how important these checks are.

I think we are in furious agreement.

But to be clear, the ground staff are the last line of defence.
They are not even that anymore because of the third party contracting and minimalist training. Probably Link SOPs are not as closely followed as Rex. Just a consequence of a small operator expanding to higher frequency RPT ops and a mish mash of flight ops culture when different operators are amalgamated.
