Standard phraseology for relaying messages?

As I am currently flying in the mountainous region of Dalmatia, it quite frequently happens to me that ATC or other ACFT stations ask me to relay a message for them. I am always a bit uneasy about this because I do not know any standard phraseology for these cases, so I have to use plain language instead.

I have checked Annex 10, PANS-ATM and Doc 9432 but could not find anything helpful.

Could somebody please tell me if there is any standard phraseology or best practice. Also, examples for both cases, ATC and ACFT stations asking to relay a message, would be great.

In the US controllers relay clearances/instructions with the prefix "ATC clears..." or "ATC instructs..." The expectation is the relaying pilot relays exactly what was just said with the included prefix.

Example "ATC clears Lufthansa four six five, climb and maintain flight level two six zero."

Or possibly,
G-ABCD from GEFGH, relay from Scottish control - then the message verbatim from the controller
