There's a standard procedure to completed via the UK CAA, it's just a form to be filled out with all the operators's details, what they'd like their R/T callsign and three letter designator to be, along with various administrative details.
This is then cross checked with ICAOs data base to ensure that the request doesn't impinge upon an existing operator's details, providing it's all OK the approval comes through.
As far as I know you have to prove that you are operating on an AOC to obtain such an approval, it's not available to private operators/non AOC holders.
