Eurocontrol Visual Performance Form

Hi,

I need to complete a Visual Performance form for Eurocontrol to confirm my eyesight is up to standard.
I originally took the form to my Optician's and had an eye test carried out, however my Optician said he was an Optometrist and not a Ophmathologist, so couldn't fill out the form.
I took the form to my Doctor today and he said the form was quite detailed and he wasn't able to complete it either because he was not qualified. However he did say this level of eyesight testing could cost up to £100 to be carried out by an Ophmathologist! - is this true?

Wondering if there is someone else who has applied to Eurocontrol who could help me out on this one, and perhaps point in the right direction as to where to go...

don't worry..the doctor didn't tell you any bull.. I paid about 75 euros to get my eyesight tested by an ophmathologist in Italy.. actually he did everything required on the visual performance form except for the field vision test cause he told me that was done seperately by another doctor so I had to spend another 35 bucks for that one. Over course these are private doctors or else you can pay a lot less if you use your health insurance but when you book, they tell you that you'll have to wait 5-6 months before you'll get the visit done if you're lucky(at least this is the situation here in italy)....

Wow. Specialists probably do cost such amounts but I've had a PM from someone here who said Tesco Opticians will be able to complete the form for a tenner! Checking it out/booking an appointment today...

Had mine done by a local opticien where I always buy my sunglasses. Therefore it was for free even. He was qualified. Mind you if you have to pay anything; if you get thru it will be worth the investment.

From Wikipedia;

By convention the term ophthalmologist is more restricted and implies a medically trained specialist. Since ophthalmologists perform operations on eyes, they are generally categorized as surgeons.
Surgeon.... NHS.... ??

And wasn't it so that the NHS is a free service in the UK?

Ended up taking my form to Specsavers yesterday - and they were able to complete it no problem.

hi all!

nothing to do with eye teste, just wondered what stage we was all at for selection / application.

I am due to start stage 2 next week just wonded if any of you will be there or if you have any tips.

Don't really know about the new tests. But relax and continu with the test. Don't give up because that is one thing an ATCO can never do.
