How do you begin an approach procedure where the required altitude at the IAF is below the MSA shown on the chart.

As an example the chart for the LLZ/DME/NDB chart for rwy10 at London City appears to show MSA's not below 2100 but the altitude at the IAF is 2000.

Once you've begun the procedure does the MSA no longer apply?

Many thanks for any info.

Once established within a procedure the procedure's altitude limits apply.

eg. A conventional 'break towards' NDB approach with a hold over the aid:

MSA or MEA applies until crossing the aid
Minimum hold altitude applies during the sector entry & hold
Published limiting step down altitudes during the approach
MDA from the last step down limit
Mandatory minimum altitudes during the missed approach
Holding minimum within a missed approach terminating hold, MSA otherwise.
