Could anyone out there help me I am after a bit of reading on the James Reason Model (Swiss Cheese).

The book that I have is called, 'Human Error' by James Reason, it is published by the Cambridge University Press and the paperback version has an ISBN number of: 0-521-31419-4.

James Reason has, I believe, written other books on related subjects so you may wish to look

His 'Managing the Risks of Organizational Accidents' (Ashgate 1997) develops the original individual error theories into how organizations can inadvertantly induce (or if mindful, can prevent) human errors. For a university psycologist, this stuff is very accessible!

An even more readable book is by Maurino, Reason, Johnston and Lee: 'Beyond Aviation Human Factors (Avebury 1995) with various detailed case studies (Erebus DC-10, Dryden F-28, the BAC-111 window etc).
