Pilots 'Under Investigation"

I work in the transport industry, not aviation, but used to.
Without making reference to any airline or individuals, would anyone mind sharing knowledge on what happens to pilots (or cabin crew) if there is a safety related investigation? Additionally, what if AAIB are involved? The reports can take a few years, I presume they are not left grounded for all that time?
If a breach of conduct is suspected, at what point does that go to discipliniary in aviation?

I am currently dealing with a very complex case and would appreciate some opinion.

Depends on the country, who you've embarrassed and if it seems you were willfully negligent or some combination of those

Usually just culture applies, if the pilot did not deliberately breach SOPs and/or endanger the safety of the aircraft or its occupants willfully, he will get remedial training at most. If he did, he might be liable to damages and probably will be dismissed. But that depends on country and company culture.

Good ideas in theory, but in practice … ‘who makes the judgement - draws the line’. Was the line drawn and clearly explained before the event?
