EAT meaning

Couple of days ago there was a slight misunderstanding between an aircraft and an ATC on the approach. Apparently vast majority of pilots and ATC think that EAT is the time of the actual approach (like passing over IAF/FAP whatever). In reality DOC4444 states it is in fact the exit time from a holding.

My question is: Is there any other document that says otherwise? We suppose that the DOC4444 definition comes from procedural times when aircraft were holding over a racetrack. Aircraft finishing the hold would directly start the racetrack approach.

Have anyone encountered this problem before?

Expected Approach Time. I.e. the time at which the aircraft is expected to leave the hold.

I've always understood it to be the time we would leave the hold, never encountered another interpretation before now.
