Metric Vs Imperial?
QNH error if it was not on an ILS but rather an RNP of some sort?

At UUDD they issue you with either QNH or QFE. At FL50 (std QNH) you then switch to metric & usually QFE. I haven't flown into UUDD for a while. It's bloody easy to do when it's busy (as it often is in Moscow). Again, I don't know, I wasn't there, but in Moscow, you're expected to join the ILS often from a 90 deg cut. It's a bit of a shock if you're used to being given headings etc ala London.
Throw in language issues, very busy RT, QNH/QFE/Metric and it's a recipe for fun and games.
In other words, I can see how it can happen.
