Incidents Attributed to Poor Briefings

Folks, I'm doing a study on effective briefings and need to collate a number of accidents and incidents where the AAIB attributed one of the major causal factors down to poor/ineffective briefing. Any links/examples much appreciated.

This old chestnut is shown at nearly every recurrent sim I go to. Poor taxy briefs and lack of SA along with very poor controlling.

Are you talking specifically about FI briefings?
