I know that not everybody is a fan, but I'm sure that absolutely all of us know and are influenced by the work of James Reason.

Just arrived on my doormat has been a catalogue of latest books on human factors, which includes his autobiography. I think that this has just gone onto my reading list, if not urgently.

Apparently the rather odd cover is because he first got interested in human error when one day in the early 1970s, he accidentally put cat food in the teapot. You now know more than I do!
