At my shop, it isn't required, but strongly recommended. Most people hand fly them.

Suggest you check your MEL. It's not mandatory however the airline I fly for has it as a required item in the MEL.

No, but following the FD is a fairly simple task. If a professional pilot can't follow a FD, perhaps it's time for him/her to consider changing careers.
