Dear all,

Can someone please tell me, if you were to fail an OPC, would this stay on your record?

In euroland the OPC stays in the company, so if you fail and then retake the assessment it will be recorded by the company, nobody else. And as far as I know, if you change company they are not allowed to disclose those information. LPC is a different story.
