I would be most grateful for any information relating to this:

Does anyone know how a country receives remuneration for provision of an ETOPS facility? I'm not sure if there is a hard and fast rule that applies to all ETOPS aerodromes/operators and whether it is similar to an overflight that if it were annotated in a flight plan then the nomination of that aerodrome would generate a fee. I assume it is something along these lines rather than an ongoing payment from the likes of ICAO?

If anyone had any specific info relating to aerodromes in the Pacific belonging to Kiribati, Tonga and Tuvalu it would be most useful as it's not a part of the world where I fly.

This all relates to the World Bank aviation infrastructure project in that region (P128938).
