Does anybody know what PAN-OPS 4 address and where I can find it.

I suspect you mean PANS-OPS, possibly then in version 4. PANS-OPS is ICAO document 8168, describing the design criterias for instrument procedures, much like the FAA TERPS.

Notice however that PANS-OPS are guidelines and not regulatory material.
