Where to find CAA list of approved FTO's?

In the following thread (Guide to obtaining a JAA PPL in the US (http://www.pprune.org/private-flying/278995-guide-obtaining-jaa-ppl-us-part-1-a.html)), the downloadable list of approved FTO's is mentioned (http://www.caa.co.uk/docs/33/SRG_FCL_APPROVEDFTOS.PDF). Unfortunately the list is no longer available on the CAA website.

I have been searching the internet extensively (google and CAA website), but was not able to find the list (or a similar list).

Does anyone have the list or a link to it?
