Is there a comprehensive document that has all ICAO standard phraseology? I am sure that picking it up piecemeal the way i have so far is leaving gaps.

ICAO Doc 9432, the Manual of Radiotelephony might be what you have in mind although it's not really what you would call comprehensive. There's more phraseology in ICAO Annex 10 too.
Sadly, though these docs may have some of the basics, R/T is something that can never be defined for every situation and develops along with experience.

Try ICAO Doc 4444, otherwise known as PANS ATM (Air Traffic Management) Chapter 12 (Phraseologies).
