Just a quickie - is the UK AIP the official source document for UK airfield approach plates and associated information? In the event that changes to an airfield facilities result in a discrepancy between the AIP and information promulgated by the airfield operator, not declared in NOTAMS, which source of information should be used?

I was always under the impression that the UK AIP (amended by NOTAM) was the definitive source of information, but I may be wrong. Can anyone help?

Yes. The Aerodrome Licensee has a legal obligation to maintain the accuracy of their AIP entry by supplement, amendment or NOTAM.
