Air Safety Report Form (ASR)

Is there a electronic version of the ASR form (UK) on the internet?

What I would prefer is one you can actually fill in then print off.

If you download CAP 382 (The Mandatory Occurrence Reporting Scheme) and look in Appendix A there is a list of forms there. It should download as a pdf file. www.caa.co.uk/docs/33/CAP382.PDF

(I know these are not actual ASRs)
Alternatively ask you company flight safety bod for an ASR form.
