Could anyone recommend a good book of threat and error management ?

blck, I do not know of any specific books for TEM.
I have several references, but their use might depend on how you view TEM or intend to use it.

Are you considering TEM for a safety management system, a strategic view? If so then the ICAO Accident Prevention Manual has some guidance.
There is more recent information in the new ICAO Safety Management Manual (http://www.icao.int/anb/safetymanagement/Documents.html). This link also has a draft document for ATC TEM.

If you require a tactical application for crew training or personal implementation, then there is a TEM model which is used to introduce Line Operations Safety Audit (LOSA). This model is somewhat limited and impractical as it is tied to LOSA and is based on academic work, see LOSA (www.e-losa.com/manuales%20OACI/Doc9803alltext.en.pdf).

Caution, there are some very large files for download, and there is a mixture of TEM masquerading as LOSA as well as operator specific views of TEM. Generally most of this TEM information is based on ‘Detect, Avoid, or Mitigate’ threats and errors, but ‘books’ showing how this is done are difficult to find.
