I suggest contacting ICAO, but they usually require money.

Try UK CAA for their manual, which in general should follow ICAO. Its free. CAP 413 Radio Telephony Manual (http://www.caa.co.uk/publications/publicationdetails.asp?id=247)

Good reply alf5071h! if memory serves me right CAP 413 Radio Telephony Manual contains the exact same text, just better gaphics and layout.

CAP 413 and the ICAO book differ in one or two places. The one that immediately comes to mind is that the UK uses 'holding point' rather than 'holding position' - although I know there are others.
