Becoming an examiner when you're already an instructor

How does an instructor become both a ground examiner, (also able to charge for specific tuition, as opposed to briefings), and also a flight examiner?

You first of all read this:

Then put in the appropriate application forms. If your particular area is already full of examiners then you may be refused on those grounds.
