How do the AAIB actually investigate an incident?

What factors are used to decide whether to just use the pilot submitted report or investigate further?

When would they start interviewing people other than the pilot, in particular 'pax'?

Know of a recent incident where an aquaintance who is a qualified pilot was a pax during an incident but wasn't asked to submit statement or even contacted prior to publication of the report...

It generally goes:

A prang by a PPL, pilot submits accident reporting form, and unless there is anything particularly dangerous or unusual, thats about as far as it goes.

Anything more serious can be followed up by inspectors, by phone, writing or in person until the inspector is satisfied that the circumstances are clear.

Fatal accidents and air transport accidents almost always get a field investigation, carried out by a number of inspectors of various disciplines.

If its really serious, an inspector's investigation is ordered.


I'd imagine your pal had a fairly inconsequential prang, therefore the aaib probs filed the report as is, and didn't need to bother the passenger. If he thought anything was seriously awry, he should pick up the phone.
