Thats a great point I think many of us forget. When the DME incident came out, I was ridiculed because I made a similar comment. Those that disagreed, said that after the first missed approach, their mindset was no longer on flying the aircraft, rather worried about the consequences of their mistake, which led to another screwed up approach and go around.
Make one thing sure, I don't disagree that the company has not manned up to taking responsibility for certain events, I only point several fingers at ourselves because apparently we once went to school for this and we have become our very own worst enemies.
