All you need is an old CAA Form FCL 150CJAR, I have some, the Examiner can sign and place in your licence.

You can only fly an EASA aircraft with LAPL privileges with that licence up to 8th April 2018.

For conversion purposes, the copy of the rating page is for current ratings, you have just renewed yours so the CAA will have to re-issue it (at a cost) based on the Examiners paperwork.
