I suspect that many pilots instinctively pull on more collective during a TR failure, exacerbating the spin and loss of control. I fully understand why perhaps only the military routinely accept the risk of training their pilots to deal with such an emergency, as it is far more likely to happen when people are shooting at your aircraft.
