Going back a few years to when BA had the daily 767 service to JFK from Manchester I believe the outbound crew positioned from LHR by flying the morning 757 shuttle up to Manchester and the inbound crew from JFK flew it back. Hope I am not making that up but it was what I was told by a member of BA crew.
