The Orlando FSDO has farmed this job out to the local Designated Pilot Examiners. There's one at Jack Brown's in Winter Haven who charges about $50 for a twenty minute process of filling in forms. There is an FAA database of DPEs

Prior to acting as pilot in command you must complete a flight review (
