Rather nice to see that the last post remembers the fact that this is a human story, as well as an opportunity to discuss objectively the points raised in the report and, of course, the myriad of questions that it raises for discussion within our community.
What I appreciate about CAA/MAA reports is that they are objective and dispassionate, unlike many of the posts within this thread - although I don't disagree with the majority of the points raised here within, I'd offer for general consideration that emotive language is neither empathetic (to those that may have known the bloke and his family) or indeed professional, if you wish to pass comment as an SME.
Of course, if you don't want or care to be either, then that's a shame but is itself testament to the many different types of personality I've worked with and continue to work with within the Aviation World.
