Changing the State of License Issue

I have couple of questions concerning my medical. At the moment I have my PPL and Medical issued by my home EASA country.

I'm going to start my CPL flight training in Poland later this year and now I want to change the the State of License Issue of my PPL to Poland.

1.
Everything seems to be clear with the PPL license but how about the Medical. Is it possible to keep my medical issued by my current home country as it is now and just send the medical documents to Polish CAA?
OR
Do I have to change my medical to Poland so that the state of issue of my Medical is then Poland?

2.
And the other question. I have done my initial Class 1 three years ago. No renewals yet so it is Class 2 at the moment. To make my Class 1 active again is it enough to do just the basic renewal? I mean that there is no need to do initial again?
