I hope that the majority of the issues have already been resolved, it sounds as if the crew are lucky to walk away from this one. That could be just the way the reporting puts the Boards findings across of course.
As I read the details of the upset, they were upset at around 15 000 and lost about 5 000 so I'll suggest the following: they were beneficiaries of at least decent, if not good, OCF training.
