There's some significant issues for the Test Team to sort out coming from that report: organisational, procedural and human factors to name a few.
I hope that the majority of the issues have already been resolved, it sounds as if the crew are lucky to walk away from this one. That could be just the way the reporting puts the Boards findings across of course.
